Medication Managesment

Abigail Pollachek, PMHNP-BC

 Abby is a dual-certified Family and Psychiatric Mental Health Nurse Practitioner. She strives to provide evidence-based psychiatric care for children, adolescents, and adults. She focuses on helping individuals manage anxiety, mood disorders, ADHD, and other mental health conditions through careful diagnostic evaluation and medication management. With training in both primary care and psychiatry, Abby strive to understand the whole person and work collaboratively to develop treatment plans that support long-term stability and well-being.  She would love the opportunity to connect and support you on your path toward feeling better and achieving lasting wellness 

   

At FamilyWorks, you will experience kind, compassionate, thorough medication management services.  These services are designed to  help clients optimize the use of their medications and improve health outcomes. Ultimately, medication management aims to ensure clients take the right medication, at the right dose, at the right time, and for the right reason. 


Medication management services include:

  • Medication Review:. A thorough assessment of all medications, including prescription, over-the-counter, and herbal remedies, to identify potential problems like drug interactions, inappropriate dosages, or unnecessary medications. 
  • Medication Regimen Development:. Based on the medication review, a personalized plan is created, which may include a medication schedule, instructions on how to take medications, and strategies for managing potential side effects. 
  • Client Education:. Clients are provided with clear and concise information about their medications, including their purpose, dosage, frequency, potential side effects, and how to store them safely. 
  • Monitoring and Follow-up:. Regular monitoring of a client's response to medication is crucial. This may involve tracking symptoms, checking for side effects, and adjusting the medication regimen as needed to ensure optimal therapeutic outcomes. 
  • Communication with Healthcare Providers:. Medication management services often involve communication with the client's primary care physician, therapist, case managers or other healthcare providers to ensure everyone is on the same page regarding the client's plan of care.

   

Psychiatric Medication Management Procedures


Thank you for choosing FamilyWorks Therapy for your mental health care. These policies help ensure safe, consistent, and high-quality treatment. Please review them carefully.

  

Medication Refills

• Please allow 48 business hours for all medication refill requests.
• No refills will be sent if you have not completed your required medication change follow-up appointment.
• Controlled medications may require more frequent appointments.
• Lost or stolen prescriptions cannot be replaced, and early refills are generally not permitted.

  

Appointments & Scheduling

• After three no-show appointments, future visits will only be scheduled before 3:00 PM.
• Medication follow-up appointments are typically required every 1–3 months, depending on clinical need.
• If you arrive more than 10–15 minutes late, your appointment may need to be rescheduled.
• Repeated cancellations or missed appointments may affect continuation of care.

  

Communication & Messaging

• Secure messages are for brief, non-urgent questions only.
• Messages sent after hours, on weekends, or holidays will be answered on the next business day.
• Concerns that require clinical assessment may result in being asked to schedule an appointment.
• We cannot provide crisis or emergency care via messaging.

  

Monitoring & Safety

• Some medications require lab work or vital sign monitoring. Continuation of medication depends on completing these tests.
• Please inform the medication management provider of any changes in medications from other providers.

  

Crisis & Emergency Situations

This practice does not provide emergency or crisis services.
If you are in crisis, feeling unsafe, or experiencing a medical emergency, please call 911, go to your nearest emergency room, or contact your local crisis hotline immediately.

  

Insurance & Payment (if applicable)

• Payment or copay is due at the time of service.
• Patients are responsible for keeping insurance information up to date.
• Any fees for missed appointments or late cancellations will follow the practice’s standard policy.
